Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately
- Support medication administration following prescribed guidelines while maintaining strict adherence to HIPAA regulations for patient privacy
- Perform patient observation to identify changes in condition and communicate findings promptly to healthcare teams
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies
- Assist with medical procedures such as catheter care and hospice support as directed by healthcare providers
- Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring accuracy and compliance
- Support patients with mobility challenges using equipment like Hoyer lifts or wheelchairs; assist with transfers safely and comfortably
- Prepare nutritious meals tailored to dietary restrictions and assist with feeding when needed; handle light housekeeping tasks including cleaning and laundry to ensure a safe environment
- Engage in home & community care activities that promote patient independence while respecting privacy and dignity
Requirements
- Proven experience as a Home Health Aide or in a similar caregiving role within assisted living facilities, nursing homes, or home care settings
- Knowledge of medical terminology, medication administration protocols, and vital signs monitoring
- Certifications such as CPR (Cardiopulmonary Resuscitation) are required; additional certifications in dementia care, hospice support, catheter care or long-term care are preferred
- Experience working with individuals with developmental disabilities or disabilities requiring specialized attention is highly valued
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Ability to work independently while following detailed care plans; excellent communication skills for collaborating with healthcare teams and families
- Physical stamina to perform caregiving tasks that involve lifting, transferring, or prolonged periods of standing and walking
- Compassionate attitude with a strong commitment to patient safety, dignity, and confidentiality under HIPAA guidelines
