CASE MANAGER JOB DESCRIPTION
SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
1. Provides assistance to Clinical Director in day to day operations
2. Monitors each patient throughout the daily schedule.
3. Enforces program rules and expectations and reports infractions to the Therapist and Clinical
4. Director.
5. Monitors patient conformance with program schedule, reports problems and informs supervisor.
6. Makes daily assignment checks and discusses results with each patient.
7. Monitors patient participation and reports problems to supervisor.
8. Resolves conflicts through general and supportive advice.
9. Participates in patient recreation, leisure, and weekend activities as scheduled.
10. Provides case management services.
11. Transports patients to scheduled activities or arranges transportation.
12. Reports any vehicle problems to supervisor.
13. Prepares educational and motivational lectures and groups.
14. Monitors AA/NA meetings on premises including recruitment of speakers.
15. Maintains the confidentiality of all patient and facility activities.
16. Complies with Policies and Procedures.
17. Accepts and utilizes supervision
18. Addresses problems noted by supervisor
19. Performs other assignments as requested by supervisor
20. Demonstrates willingness to accept responsibility
21. Completes all tasks with concern for work quality
Salary : $40,000 - $50,000
