Position Overview
NPORT Staffing is seeking compassionate and skilled Physical Therapist (PTs) to provide high-quality care in homecare and hospice settings. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact on patients' lives by helping them maintain mobility, comfort, and dignity in their own homes.
We offer the option to work as a W-2 employee or 1099 independent contractors, depending on your preference.
Key Responsibilities
- Evaluate and assess patients' physical conditions, functional limitations, and rehabilitation needs
- Develop and implement an individualized treatment plan tailored to homecare and hospice patients.
- Provide therapeutic exercises, mobility training, and pain management interventions
- Educate patients, families, and caregivers on safe mobility, fall prevention, and care techniques
- Collaborate with interdisciplinary teams including nurses, physicians, and other healthcare professionals
- Accurately document patient progress and maintain compliance with regulatory and company standards
- Adjust treatment plans based on patient progress and evolving needs
Qualifications
- Active Physical Therapist license in the state of practice
- Graduate of an accredited physical therapy program
- Minimum of 1 year of experience preferred (homecare or hospice experience is a plus)
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Reliable transportation and valid driver's license
