A tech-enabled medical group in Brooklyn, New York is seeking an experienced NP or PA (AGACNP, FNP, or AGPCNP) to deliver high-acuity acute care directly in patients' homes. This is a field-based role that takes your ED, inpatient, or post-acute clinical skills out of the facility and into the community, performing hands-on interventions, ordering and interpreting diagnostics, and building holistic care plans for some of the most vulnerable patients in New York. Three 12-hour shifts per week means four full days off every week, and a sign-on bonus rounds out one of the stronger total compensation packages in home-based acute care in the New York metro.
Key Responsibilities
- Perform problem-focused assessments and hands-on interventions in the home: IV fluids, wound care, splinting, medication administration
- Order and interpret diagnostics including labs, EKGs, and POCUS
- Build individualized care plans in collaboration with referring clinicians honoring patient goals and palliative principles
- Prevent hospitalizations and reduce readmissions through proactive, relationship-based acute care
- Partner with a dedicated team of paramedics, nurses, and physicians on every case
- Participate in quality improvement initiatives with measurable outcome targets
- Maintain accurate and timely clinical documentation
Qualifications
- Active, unrestricted New York NP or PA license
- AGACNP, FNP, or AGPCNP credential
- Minimum 2 years (3,600 hours) experience in ED, inpatient medicine, or post-acute care
- Current BLS required | ACLS preferred
- EMS background and IV access/field medication experience a strong plus
- Valid driver's license required | Ability to carry equipment up to 60 lbs and navigate stairs
- Comfortable in fast-paced, startup, and field-based clinical environments
Schedule & Compensation
- $176,800–$197,600/year sign-on bonus
- Full-time | Three 12-hour shifts/week | Four consistent days off
