A geriatric and medically complex care practice in Brooklyn, New York is looking for an experienced NP (AGACNP, AGPCNP, or FNP) who wants to practice at the highest level of advanced practice medicine — managing complex older adults across primary care, home hospital medicine, and hands-on wound care in a continuous, relationship-based care model that actually changes patient outcomes. This is not a high-volume, low-acuity role. It's a substantive, intellectually demanding position for an NP with real hospital medicine depth, wound care proficiency including debridement, and the clinical confidence to manage high-acuity patients outside traditional inpatient walls.

Key Responsibilities
- Manage a geriatric, multi-comorbidity patient panel across chronic and acute care
- Deliver acute care services including home hospital medicine for higher-acuity patients
- Perform comprehensive wound care — assessment, treatment planning, ongoing management, and debridement
- Complete H&Ps, order and interpret diagnostics, and build and adjust treatment plans
- Prescribe and manage medications including controlled substances
- Coordinate with clinical team, specialists, and families for safe transitions and continuity of care
- Maintain accurate, timely documentation per Medicare and payer requirements
Qualifications
- Active New York State NP license (required)
- Current DEA registration (required)
- Active Medicare credentialing/enrollment (required)
- Board certified FNP, AGPCNP, or AGACNP
- Hospital medicine experience (required)
- Wound care experience including debridement (required)
- 5 years NP experience preferred — hospital medicine and critical care background strongly preferred
- 1–2 years dedicated wound care experience
- Applicants with 2 years NP experience plus substantial geriatric/wound care RN background encouraged to apply
- Critical care/ICU, home-based care, and geriatric/multi-comorbidity experience a strong plus
Schedule & Compensation
- $200,000–$250,000/year (commensurate with experience)
- Full-time | Five 8-hour shifts | Onsite Brooklyn, NY
