A young primary care organization in Washington, DC is hiring a Lead FNP to anchor four small clinics embedded inside DC public housing communities — serving predominantly Medicaid families across the full lifespan with thirty-minute appointments, onsite labs, and the kind of continuity that actually builds trust. Three days onsite, two days telehealth, Monday–Friday, no weekends. Full practice authority from day one under DC's full scope licensure. This is a founding team clinical leadership role — you'll carry your own panel, mentor the clinical team, and help shape the care model as the organization grows.


Key Responsibilities — Direct Patient Care

  • Deliver comprehensive primary care across the lifespan including well child visits, immunizations, physicals, and screenings
  • Diagnose, treat, and manage acute and chronic conditions with emphasis on chronic disease management and prevention
  • Manage complex patients with multiple chronic conditions and significant social needs
  • Prescribe and manage medications including controlled substances
  • Coordinate care with specialists, hospitals, and community organizations
  • Deliver care through both onsite visits and telehealth


Key Responsibilities — Clinical Leadership

  • Serve as senior APP and clinical resource for the team
  • Provide clinical coverage and decision-making authority when executive leadership is unavailable
  • Develop clinical workflows, care pathways, and operational processes as the model matures
  • Mentor NPs, nurses, MAs, and care coordinators
  • Contribute to recruitment, onboarding, and training of new clinical staff


Key Responsibilities — Population Health

  • Lead chronic disease management and care gap closure across the patient panel
  • Support quality improvement, documentation quality, and performance measurement
  • Address social determinants of health through community and housing partnerships
  • Contribute to value-based care and population health data collection
  • Participate in occasional community outreach events at clinic sites


Qualifications

  • Active, unrestricted Washington, DC NP license (or ability to obtain before start)
  • FNP certification through ANCC or AANP (required — panel includes pediatric patients; AGPCNP does not meet requirement)
  • Active DEA registration required
  • MSN, DNP, or equivalent advanced nursing degree
  • 5 years NP clinical experience
  • Demonstrated chronic disease management experience
  • Experience serving Medicaid or comparably underserved populations
  • Comfortable seeing pediatric patients — well child care, vaccines, physicals
  • EHR proficiency required
  • Leadership, program development, or quality improvement experience preferred
  • Startup or build-phase clinical environment experience preferred


Schedule & Compensation

  • $125,000–$145,000/year (set by experience)
  • Full-time | Monday–Friday | No weekends
  • Hybrid — 3 days onsite / 2 days telehealth from home
  • ICHRA — $300/month toward individual health plan
  • PTO up to 2 weeks accrued 7 paid holidays | Paid sick leave from day one
  • CEU allowance up to $500/year | Malpractice covered
