General Description
This position plays an important role in promoting health and well-being for older adults by conducting nutrition program eligibility assessments, supporting participant access to services, and coordinating nutrition education and health promotion activities. The Nutrition & Health Promotion Coordinator works directly with participants and community partners to support access to nutrition services and encourage healthy aging.

Essential Duties
  • Conduct in-person assessments to determine eligibility for home-delivered and congregate meal services in accordance with program requirements.
  • Complete reassessments as required and maintain accurate participant documentation and records.
  • Track participant eligibility and reassessment schedules to support continuity of services.
  • Identify participant needs or concerns and connect individuals with Aging and Disability Resource Connection (ADRC) Specialists or other community resources as appropriate.
  • Conduct community outreach and collaborate with program partners to support nutrition program goals.
  • Coordinate and support activities related to nutrition education, health promotion, physical activity, and self-management of chronic conditions.
  • Build positive working relationships with participants through respectful communication, reliability, and follow-through.
  • Plan, coordinate, and deliver nutrition and health promotion education in accordance with federal and state program standards.
  • Maintain accurate records and reporting for nutrition and health promotion activities.
  • Follow Agency policies, confidentiality requirements, and mandatory reporting obligations.
Minimum Qualifications
A qualified applicant will have a minimum of three (3) years of equivalent combination of education and/or experience which demonstrates the knowledge, skills and abilities required. The following qualifications meeting the minimum requirements will be considered:
  • High school diploma or GED
  • Experience in social services, nutrition services, eligibility determination, or community program coordination
  • Experience working with older adults and/or individuals with disabilities in a social service setting
  • Experience conducting participant assessments and maintaining accurate documentation
Other Requirements
Driving for Agency Business. Positions that require driving must possess a valid driver’s license and maintain an acceptable driving record. May require proof of personal automobile liability insurance in accordance with ORS Chapter 806 and use of a personal vehicle.
Background Check. Employment is contingent upon successful completion of a criminal history check. Conviction of a crime does not automatically disqualify an individual from employment. Some positions may require additional screening, including but not limited to state or national fingerprint-based background checks.
Travel. Ability to travel throughout the Agency service area.

Knowledge, Skills, and Abilities
  • Knowledge of or ability to learn eligibility requirements for nutrition services.
  • Ability to conduct in-person assessments and identify participant needs.
  • Knowledge of services and community resources available to older adults and individuals with disabilities.
  • Ability to maintain accurate records, documentation, and data.
  • Experience with electronic data systems and basic computer applications.
  • Strong organization and time management skills.
  • Effective communication and relationship-building skills with participants, staff, volunteers, and community partners.
  • Ability to work independently and use sound judgment in a variety of situations.
Work Environment and Physical Demands
  • Work is performed in participant homes, community settings, and office environments.
  • Frequent travel throughout the assigned service area.
  • Regular use of a laptop computer and telephone.
  • Frequent walking, standing, talking, and listening throughout the work day.
  • Occasional bending, reaching, stooping, pushing, pulling, and lifting up to 30 pounds.
  • Driving an Agency or personal vehicle as needed for program responsibilities.
  • Interaction with diverse populations in homes and office settings, including occasional exposure to contagious illness, challenging behavior, or domestic animals.
