What You’ll Do

As a CLS Worker in the BHH program, you’ll provide hands-on support to individuals with mental health challenges, intellectual/developmental disabilities, and chronic health conditions. Your responsibilities will include:

• Supporting individuals in recreational, volunteer, and community-based activities
• Assisting with daily routines and goals outlined in the Individual Plan of Service (IPOS)
• Promoting health, safety, and independence in the least restrictive setting
• Attending weekly BHH care team meetings to coordinate care
• Facilitating health education groups and wellness initiatives
• Providing education on chronic conditions and self-management strategies
• Tracking referrals and connecting individuals to community resources

What You Bring

• Certified Medical Assistant (CMA) or medical assistant training preferred
• High school diploma or GED preferred
• Knowledge of intellectual/developmental disabilities and mental health issues is helpful
• Strong communication skills and a collaborative spirit
• Lived experience with mental illness, developmental disabilities, or substance use is valued
• Must be at least 18 years old
• Valid Michigan driver’s license and acceptable driving record
• Ability to work flexible hours, including evenings, weekends, holidays, and occasional overnight trips
