SUMMARY OF RESPONSIBILITIES:
Case managers will provide those services, which will assist individuals who have a serious mental illness, Intellectual/Developmental Disability, autism spectrum disorder (ASD) (adults), serious emotional disturbance (children), or substance use disorder in accessing needed medical, social, educational, and other services. Case manager will assist individual in accessing and maintaining a continuum of mental health services, in order that they can continue to remain as independent as possible, as identified through person-centered planning. Core elements of case management services are assessment, service plan development, linking/coordination of services, reassessment/follow-up, and monitoring of services.

Rate of Pay: $21.83 - $30.09

FLSA: Non-Exempt/Full-Time

Reports To: Case Management Supervisor

QUALIFICATIONS:
Education: Nursing Degree or Bachelor's Degree in education, social work, and have appropriate certification, licensure, or registration as required. Possess State Registration as a Registered Nurse, LBSW (or be able to acquire within a reasonable period of time i.e., LLBSW will be eligible for an LBSW).

Experience: One or more years of relevant mental health or relevant nursing experience. Experience less than one year for a Qualified Intellectual Developmental Disabilities Professional (QIDP) and Qualified Mental Health Professional (QMHP) or three years of experience as Child Mental Health Professional (CMHP) will entail additional clinical supervision until minimum experience is met. Lived experience with mental illness, intellectual and developmental disabilities, and/or substance use disorders valued.

Other Good communication skills (both verbal and written) are essential. Functional computer literacy is required within 6 months from the date of hire. Must possess a valid driver's license and acceptable driving record (as determined by the agency).

ESSENTIAL JOB FUNCTIONS:

  • Demonstrates knowledge of universal precautions, standards of practice, including but not limited to confidentiality and recipient rights, and emergency protocol.
  • May be assigned to service as an active member of the Children's Diagnostic and Treatment Team. Including annual continuing education training specific to children (24 CE hours according to Policy 5135).
  • Demonstrates understanding of assessment and diagnosis for persons requesting Mental Health Services in accordance with the latest editions of the DSM and ICD.
  • Demonstrates effective case recording in accordance with Person Centered Planning, and the Agency Case Recording Protocol.
  • Have positive work relationships and able to work as an effective team member, and develop those relationships that are needed to support the person.
  • Provide thorough assessment and service in the least restrictive environment.
  • Provide crisis intervention to persons served as needed. Will participate in the agency's emergency on-call rotation.
  • Collaborate with other staff in assessing person served's needs and developing person-centered and family-focused treatment plans.
  • Directs the work of the CLS staff, inclusive of in-servicing and monitoring services and supports on a regular basis (at least quarterly).
  • Works with all programs to support people in getting the needed supports, as authorized in the person-centered plan.
  • Consult with various agencies, educational settings, individuals, and organizations as needed. Will work with county jails when the person served is incarcerated, providing jail services as appropriate.
  • Will demonstrate functional computer literacy necessary for the timely completion of required documentation according to the Agency Case Recording Protocol.
  • Will be required to maintain the highest degree of state licensure/registration specific to this position.
  • Staff will be expected to participate in trainings/in-services aimed at the provision of services for persons served with co-occuring disorders and increasing fidelity to best practice.
  • May co-faciliatate group therapy services.
  • Attend meetings as required.
  • Maintains effective communication and favorable relationships with individual served, clinical staff, support workers, families, educational facilities and other agencies and entities.
  • Write programs and monitor their implementation and efficacy.
  • Work must comply with guidelines established by CARF, the Michigan Department of Health and Human Services (MDHHS) and any other regulatory/monitoring agencies that protect persons served. Will meet productivity standards set by the administrative leadership.

NON-ESSENTIAL JOB FUNCTIONS:

  • Performs other related duties as required.
  • Engages in active self-care.
  • May be requested to be involved in the Board's continuous quality improvement program.
  • May be required to provide own transportation while performing agency-related duties.

Ability to walk, bend, stand, sit, lift up to 25 pounds, with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive.
