Responsibilities:
- Assists Admissions Coordinator with all new admissions, re-admissions, discharges, and final discharges to and from the facility.
- Give tours of the facility to prospective residents and/or responsible parties who are seeking NCSC placement when admissions coordinator is unavailable.
- Supervises Social Work staff.
- Case management of forty residents
- Meets with each new resident and/or responsible party of assigned unit to complete Social History
- Completes on going documentation of information pertinent to each resident’s case (i.e., quartile and annual reviews, initial and quarterly MDS form, adjustments to long term care, screens, etc.)
- Make sure residents’ psychosocial needs are met. Utilizes social work interventions to meet resident needs appropriately.
- Attends care conferences for assigned residents on a weekly basis.
- Assists in monitoring that residents’ material needs are adequately met.
- Provides on going direct Social Work services to residents and responsible parties based on assessed needs. Monitors and assesses each resident’s adjustment to the facility and responsible party’s acceptance of placement.
- Coordinates between departments room transfers as indicated or requested by the residents, party responsible, staff members, etc.
- Maintains a separate social work file for each assigned resident in the social work office.
- To be available on a rotating on-call basis.
- Advocates on behalf of the residents and their needs as related to Quality-of-Life issues.
- Coordinates and facilitates Family Council meetings and significant other meetings on a quarterly basis or as scheduled.
- Coordinates and attends (along with Activity Director), Resident Council meetings held monthly if requested by Resident Council.
- Oversee the complaint and recommendation process of the facility. Consults with Administration on a regular timely basis regarding these issues.
- Liaison between Administration, Business Office, Physicians, and other departments on behalf of the residents and/or responsible party.
- Attend department head meetings.
- Coordinates monthly staff meetings.
- Screens possible new residents as necessary
- Writes and implements new policies and procedures as applicable to the Social Work Department
- Updates current policies and procedures as applicable to the Social Work Department
- Represents the facility in the community by actively participating in related professional and community organizations, correspondence.
- Presents a mandatory in services on at least an annual basis on Resident Rights
- Knowledge of community service and agencies for referrals as applicable to the residents and/or family need (i.e., support groups, financial services, etc.)
- Attend seminars and conferences in the community related to the needs of the residents and to promote professional and personal growth.
- Maintains an active resource file on services and agencies available in the community. This file will also include pamphlets and/or booklets, which may be beneficial to residents, and responsible parties as related to advocacy, support groups, related organizations, home care companies, adaptive equipment, and housing options.
- Coordinates Hospice and attends Hospice meetings, as necessary.
- Coordinates and assists social work in forming specialty groups.
Requirements:
- Minimum BS/BA Degree in Social Work on or related field. Masters preferred
- Excellent work ethic
- Must have demonstrated ability to make decisions with good nursing judgment, handle emergencies and maintain excellent patient assessment skill.
- Must be able to move intermittently throughout the work day.
- Must be able to speak and write the English language in an understandable manner.
- Must be able to cope with the mental and emotional stress of the position.
- Must be able to see and hear or use prosthetics that will enable these senses to function adequatelyto assure that the requirements of this position can be fully met .
- Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
- Must be in good general health and demonstrate emotional stability.
- Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times, hostile people within the facility. Must be ableto lift, push, pull and move a minimum of 50 pounds. Must be able to assist in the evacuation of residents.
- At least one year of LTC experience.
