Director of Social Work

The ideal candidate will be a professional, compassionate and outgoing individual with work experience in the social services field, as well as experience in the skilled nursing/long-term care setting. The Director of Social Work will plan, develop, organize, implement, evaluate, and direct social service programs in accordance with current existing federal, state, and local standards (as well as established facility policies and procedures), to assure that the medically related emotional and social needs of the residents are met and maintained on an individual client basis.Requirements:Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in directing the overall operation of the facility's delivery of services.Performs resident evaluations as required by state regulations.Long term care/ sub-acute and discharge planning experience requiredSupport state regulations for admission and retention of residents.Assists residents and families with the transition process.Orients residents and families to the facility.Assists and interacts with residents and families to access services.Discharge planning and assistance experience for elderly populations.Participates in development and implementation of each resident's individual care plan.Communicates with other members of the management team to ensure coordination of services to residents.Documents case management notes in residents' files as necessary.May perform other duties as assigned.Knowledge of MDS is essential.Qualifications:MSW or LMSW is required.Experience in the field of social work providing services to an adult population as required by state regulation if applicable.Two to five years experience in social work providing services to an adult population is preferred.Basic computer knowledge, particularly in Microsoft Word and ExcelLicensed as required by state regulations if applicable.Valid driver's license.
