Senior Community Support Specialist, North Suffolk Community Services,

Inc. (Boston, MA): Under the clinical supervision of the Program

Director and Clinical Coordinators, the Senior Community Support

Specialist will participate in the engagement, assessment and treatment

planning process and as part of the team, provide interventions to

address needs and goals of persons served. Provide direct services to

people in community and GLE settings in the areas of housing, risk

mitigation, transportation, skill-building, money management, medication

administration and other areas based on the person centered treatment

plan. Engage people and partner with them to improve their health and

wellness, achieve symptom stabilization and self-management, develop

and/or restore skills impacted by mental illness and live a

self-directed life. Assist people with psychiatric and substance use

disorders to identify personal priorities, preferences, strengths, and

interests in order to help them establish goals that support a life in

recovery. Provide direct care services in timely and flexible ways.

Collaborate with persons to help them to write goals with specific

action steps in order to develop effective treatment/rehabilitation

recovery plans. Participate in the development and implementation of

Safety Plans and Crisis Prevention and Response Plans in order to help

prevent and mitigate risk factors. Use evidence based practices

including motivational interviewing as directed by clinical supervisors.

Assist in provision of skill building interventions that help move a

person on to the most independent setting or level of care. Advocate for

better access to public services and natural supports for persons in

order to facilitate their recovery and full integration into the

community. In collaboration with Clinical Coordinators and other members

of the Integrated Team, participate in comprehensive assessment of

psychiatric history (e.g., onset, course and effect of illness, past

treatment and responses, and risk behaviors), mental status, and

diagnosis; physical health and dental health; use of drugs or alcohol;

education and employment; social development and functioning; activities

of daily living (e.g., self-care, living situation, nutrition, money

management); and family structure and relationships. Gather information

about community agencies and families to assist staff in the provision

of services. Document each persons progress in the confidential record

according to established methods and procedures. Carry out the assigned

interventions and action steps of the treatment plan, through the

provision of direct support and rehabilitation services to people on an

individual, group, and family basis (in conjunction with professional

staff) in the office and in community settings to teach behavioral

symptom management techniques to alleviate and manage symptoms not

reduced by medication and to promote personal growth and development by

assisting people to adapt to and cope with internal and external

stresses. Provide interventions to assigned persons in GLE settings.

Basic annual pay is \$61,340.03. Minimum requirement: Bachelor\'s degree

in Exercise and Health Science or a closely related field. Qualified
