🏡 Job Summary

As a Physical Therapist (PT), you'll evaluate patients, create and carry out treatment plans, and coordinate with our rehab team to provide top-tier care in the home setting.

🛠️ Key Responsibilities

• Evaluate patient’s condition and functional level
• Develop and implement treatment plans
• Supervise PTAs and HHAs per plan of care
• Provide in-home therapy and adaptive equipment guidance
• Maintain accurate and timely documentation
• Ensure patient and family education for home programs
• Submit notes/visits in a timely manner
• Adhere to HIPAA confidentiality standards

📋 Requirements

• Active PT license in Ohio
• 1 year of PT experience (preferred)
• OASIS and WellSky/Kinser experience (strongly preferred)
• Physical exam (within 6 months of start)
• Current TB test and CPR certification
• Valid driver’s license and auto insurance
• Ability to attend orientation in Dublin, OH
