Full-Time | Park Manor Health & Rehabilitation | Northport, AL

Base Pay: $25.00-$29.55 per hour (based on experience)

Shifts Available: Day Shift, Evening Shift, and Night Shift

Park Manor Health & Rehabilitation is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our skilled nursing and rehabilitation team. As a Licensed Practical Nurse (LPN), you'll provide high-quality nursing care in a long-term care and rehabilitation setting while working alongside an experienced interdisciplinary team committed to helping residents achieve their highest level of independence and quality of life.

Licensed Practical Nurse (LPN) Responsibilities

• Provide direct nursing care to residents in a skilled nursing, rehabilitation, and long-term care setting.
• Assess, monitor, and document changes in resident conditions.
• Administer medications, treatments, and nursing interventions according to physician orders.
• Monitor resident response to medications and treatments.
• Collaborate with Registered Nurses (RNs), physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
• Accurately complete electronic nursing documentation.
• Respond promptly to changes in resident condition and communicate concerns appropriately.
• Supervise and support Certified Nursing Assistants (CNAs) to ensure quality resident care.
• Ensure compliance with state and federal regulations and facility policies.
  
  

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing.
• Current CPR/BLS certification.
• Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric nursing is preferred but not required.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work collaboratively within an interdisciplinary healthcare team.
