Job Description
Member needs hands-on support due to limited mobility and weakness on one side, with added back and leg pain that can make movement difficult. Caregiver must be comfortable assisting with safe transfers, repositioning, and mobility support while helping reduce fall risk. Member also needs dependable help with incontinence care and maintaining comfort throughout the morning routine. Ideal caregiver is patient, attentive, safety-focused, and experienced with mobility assistance, personal care, and monitoring for changes in condition (reporting concerns to the agency/family as appropriate).

Required Duties
Personal Care Assistance: bathing support, grooming, dressing, toileting/incontinence care, skin care observation, and hygiene set-up.
Mobility & Safety Support: assist with transfers, walking/mobility support as tolerated, repositioning for comfort, and fall-prevention practices.
Meal Support: meal prep and light cooking, assistance with eating as needed, and clean-up after meals.
Light Housekeeping: tidying, laundry, changing bed linens, cleaning commonly used areas, and maintaining a safe walkway/free of trip hazards.
Medication Reminders: reminders only (no administering).
Observation & Reporting: monitor for changes in breathing, fatigue, swelling, pain, or overall condition and report concerns per agency policy.
Transportation/Errands: only if requested later—driving may be required for essential errands such as shopping or appointments (if added to the care plan).
