Position Overview
We are seeking a dedicated and compassionate Case Manager to join our Assertive Community Treatment (ACT) Team. The ACT Team Case Manager plays a critical role in delivering intensive, community-based mental health services to individuals with severe and persistent mental illness. This position involves working collaboratively within a multidisciplinary team to support clients in achieving stability, independence, and improved quality of life.
Key Responsibilities
- Provide direct, community-based services to clients, including home visits and outreach
- Develop, implement, and monitor individualized treatment plans in collaboration with clients and the ACT team
- Assist clients with daily living skills, housing stability, employment, and social integration
- Coordinate care with internal team members and external providers, including healthcare, social services, and family supports
- Monitor client progress and document services in accordance with agency and regulatory standards
- Provide case management using motivational interviewing to help clients to utilize their strengths and abilities to address their problem
- Address risk factors such as substance use, poor use of leisure time, and self-doubt by applying problem-solving services
- Promote client benefits and income support by escorting consumers to appointments for Medicaid, food stamps, SSI/D, HRA, child support etc.
- Provide crisis intervention and support as needed
- Participate in team clinical meetings, case conferences, and on-call rotation as required
- Other duties as assigned
Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (Master’s preferred)
- Minimum of 6 months experience working with individuals with serious mental illness
- Knowledge of community resources and recovery-oriented practices
- Strong communication, organizational, and problem-solving skills
- Ability to work independently and as part of a multidisciplinary team
Preferred Qualifications
- Experience with ACT or other intensive community-based programs
- Bilingual skills are a plus
- Familiarity with electronic health records (EHR) systems

Salary : $55,000 - $60,000
