What are the responsibilities and job description for the ACT Team Case Manager position at The Guidance Center?
Job Overview: The ACT Team Case Manager functions as an integral part of the Assertive Community Treatment Team to provide intensive, recovery-based treatment for the highest risk consumers of mental health resources. The ACT Team Case Manager serves as a primary case coordinator in order to ensure each client enrolled in ACT is receiving the appropriate services relevant to their recovery goals.
Responsibilities and Duties:
- Participate in all aspects of client’s treatment from admission through discharge; serve as a primary source of service coordination for all clients enrolled in ACT;
- Work collaboratively as part of the ACT Team in order to provide comprehensive care that meets the needs of client team members;
- Respond to and assess emergency situations and incidents; assure that appropriate actions are taken to protect the health and welfare of clients and their families;
- Understand and be capable of performing the functional duties of other staff ACT Team members and be willing to serve as a substitute for the provision of other specialist services when the designated staff team member is unable to do so;
- Attend daily team meetings, individual supervision, and group supervision as scheduled;
Education and Experience:
Candidates must have the following qualifications:
- Bachelor’s Degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education required
- Experience working in a behavioral health setting strongly preferred
