The Case Manager on the ACT Team provides comprehensive, community-based case management services to adults with severe and persistent mental illness. The Case Manager works collaboratively with a multidisciplinary ACT Team to deliver person-centered care. The person selected for this position will have a minimum of a Bachelor's degree in a social sciences field. Requires a valid driver license with clean driving record and reliable transportation.
