About the Role

New Freedom is seeking a Continuity of Care Coordinator to support members as they transition between levels of care and into community-based services. This role plays a critical part in ensuring safe, smooth, and successful discharges by coordinating placements, removing barriers, and connecting members to the resources they need for continued recovery.

You’ll work closely with internal care teams and external partners to help individuals navigate housing, treatment programs, and community services—all while maintaining a trauma-informed and member-centered approach.

What You’ll Be Responsible For

  • Coordinate member transitions, facilitate discharge meetings with members, provide guidance on next steps, timelines, and resources
  • Support placement and resource navigation by coordinating placements into treatment programs, stabilization services, shelters, and housing resources
  • Connect members with community-based services, including resource fairs and other support networks
  • Collaborate across teams, by partnering with Clinical Services, Transitional Services team members, Care Coordination, and other stakeholders
  • Act as a liaison with community providers and external agencies
  • Track and document care plans, maintaining accurate and timely documentation in the Electronic Health Record (EHR)
  • Monitor discharge timelines, placement outcomes, and follow-up needs
  • Maintain resources and improve processes, keeping referral databases, housing directories, and community resources up to date
  • Research new placement options and develop community partnerships
  • Additional duties as assigned

What We’re Looking For

  • High school diploma or GED, preferably an Associate’s or Bachelor's degree in a related field
  • Behavioral Health Technician (BHT) certification or qualifications of a BHT
  • Strong communication, organization, and problem-solving skills
  • Ability to work both independently and as part of a team
  • Comfortable working in a fast-paced, evolving environment
  • Proficiency with Microsoft Office and electronic documentation systems (EHR)
  • Valid Arizona driver’s license
  • Ability to pass background check, drug screening, and TB testing
  • Four or more years of experience in behavioral health, case management, or a related field preferred
  • Experience with discharge planning, care coordination, or housing placement preferred
  • Peer Support certification preferred
