Job Title: LPN/LVN – Long Term Care (LTC)
Location: New Lisbon, WI
Duration: 13 Weeks
Shift: Full-Time PM Shift (2:00 PM – 10:00 PM), Every Other Weekend
Position Summary: We are seeking a dedicated Licensed Practical Nurse (LPN/LVN) to provide quality nursing care in a Long-Term Care setting. The role involves administering medications, monitoring resident health, and ensuring effective communication with healthcare teams, residents, and families.
Key Responsibilities:
- Administer medications following the “Five Rights” of medication administration
- Monitor residents and report any changes in condition to the Charge Nurse
- Communicate significant changes in resident status to physicians and family members
- Report any concerns related to abuse, neglect, or misappropriation to leadership
- Assist Certified Nursing Assistants (CNAs) with resident care and call lights as needed
- Maintain accurate documentation and ensure clear communication among care teams
- Perform additional duties as assigned
Required Qualifications:
- Active Wisconsin Licensed Practical Nurse (LPN) license
- Current BLS/CPR certification
- Long-Term Care experience preferred
- Strong attention to detail and accuracy
- Excellent communication and teamwork skills
