Duration: 13 Weeks
Pay Rate: $28 – $32 per hour
Job Overview
"Navitas Healthcare, LLC" is seeking an experienced Licensed Practical Nurse (LPN / LVN) to support a Behavioral Health – Dementia Care Unit in Newark, NY. The LPN will provide compassionate, patient-centered care to individuals with dementia and behavioral health needs while maintaining a safe and supportive clinical environment.
Schedule
Shift: Mid Shift
Hours: 2:00 PM – 10:00 PM
Schedule: 5 shifts per week (8 hours each)
Weekend Requirement: Every other weekend
Block Scheduling: Not available
Floating: May be required based on facility needs
Key Responsibilities
Provide direct nursing care in a behavioral health and dementia care setting.
Monitor and document vital signs, intake/output, and patient conditions.
Assist with behavior management and apply de-escalation techniques when required.
Perform blood glucose monitoring and specimen collection/labeling.
Follow isolation precautions and infection control protocols.
Support patients experiencing challenging or assaultive behaviors using approved intervention techniques.
Collaborate with physicians, RNs, and interdisciplinary healthcare teams to ensure optimal patient outcomes.
Maintain accurate patient documentation using EPIC electronic health record (EHR) system.
Required Qualifications
Active LPN / LVN License
Minimum 2 years of nursing experience
BLS (Basic Life Support) Certification
Experience in Skilled Nursing Facility (SNF), Nursing Home, or Behavioral Health settings
