"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long Term Care (LTC) facility in Rushford, MN. This role focuses on delivering high-quality, patient-centered care while collaborating with residents, families, and the healthcare team to ensure optimal outcomes.
Key Responsibilities- Administer medications and treatments as prescribed by physicians
- Monitor residents’ health status and promptly report changes to the RN or physician
- Provide hands-on patient care, including wound care, catheter management, and assistance with activities of daily living (ADLs)
- Accurately document care and patient information in electronic medical records (EMR)
- Collaborate with Certified Nursing Assistants (CNAs) and interdisciplinary team members
- Adhere to infection control standards, safety guidelines, and facility policies
- Offer emotional support and advocate for residents’ well-being
- Active LPN license in Minnesota or a compact state
- Minimum of 1 year of experience in Long Term Care or Skilled Nursing Facility (SNF)
- Current BLS certification (American Heart Association preferred)
- Strong clinical judgment, attention to detail, and adaptability to varying schedules
