Key Responsibilities
- Identify and cultivate referral sources within hospitals, SNFs, and other healthcare settings.
- Collaborate closely with internal intake and clinical teams to support patient transitions and care planning.
- Educate healthcare professionals on Upstate Homecare’s enteral services and coordinate care solutions tailored to each patient.
- Act as a patient care advocate and resource for families and healthcare providers.
- Maintain regular, professional communication with referral partners and internal departments.
- Track referral activity and maintain accurate records for performance and growth analysis.
- Prior sales or marketing experience preferred, especially in healthcare or medical services.
- Excellent verbal and written communication skills; comfortable presenting to individuals and groups.
- Strong organizational skills with the ability to prioritize, multitask, and work independently.
- Proficient with computers and common business software.
- Knowledge of home medical equipment (HME) or infusion reimbursement is a plus.
- Must possess a valid driver’s license and have reliable transportation.
- Community-based role with frequent travel to hospitals, SNFs, and healthcare facilities.
- Exposure to variable weather conditions and typical road travel risks.
