Position Summary

The Paramedic – At-Home Care Program provides high-quality, patient-centered clinical care in the home setting as part of a mobile integrated health or community paramedicine model. This role supports patients who benefit from in-home assessment, care coordination, chronic disease monitoring, post-discharge follow-up, prevention education, and connection to appropriate health and community resources. The Paramedic works under established medical direction, protocols, and organizational policies to improve patient outcomes, reduce avoidable emergency department visits, and support safe care transitions.
 
Essential Duties and Responsibilities
  • Conduct scheduled and as-needed home visits for enrolled patients in accordance with program guidelines, care plans, and medical direction.
  • Perform patient assessments, including vital signs, focused physical assessments, medication review, home safety observations, and identification of changes in condition.
  • Provide clinical interventions within scope of practice, approved protocols, and physician or advanced practice provider orders.
  • Support post-hospital discharge follow-up, including monitoring symptoms, reviewing discharge instructions, identifying barriers to recovery, and escalating concerns as appropriate.
  • Provide education to patients and caregivers related to chronic disease management, medication adherence, fall prevention, nutrition, emergency warning signs, and use of available resources.
  • Coordinate with physicians, nurses, case managers, social workers, pharmacists, behavioral health providers, EMS, and other members of the care team.
  • Assist with referrals and patient navigation to primary care, specialty care, social services, behavioral health, transportation, and community resources.
  • Document all assessments, interventions, patient education, care coordination activities, outcomes, and follow-up needs accurately and timely in the designated record system.
  • Identify urgent or emergent needs and initiate appropriate escalation, including contacting medical direction, activating EMS response, or facilitating transport when indicated.
  • Maintain equipment, supplies, medications, vehicle readiness, infection control standards, and patient confidentiality requirements.
  • Participate in quality improvement, case review, program development, training, outreach, and data collection activities.
  • Comply with applicable federal, state, and local regulations; scope-of-practice requirements; HIPAA; organizational policies; and clinical protocols.
     
Required Qualifications
  • Current and unrestricted Paramedic license or certification in the state of practice.
  • Current Basic Life Support certification.
  • Current Advanced Cardiovascular Life Support certification.
  • Valid driver’s license with an acceptable driving record, if travel to patient homes is required. Must have and maintain an active and unrestricted Driver’s License.
  • Demonstrated knowledge of emergency medical care, patient assessment, clinical documentation, infection prevention, and patient safety practices.
  • Ability to work independently in the field while maintaining communication with medical direction and the interdisciplinary care team.
  • Strong interpersonal, communication, de-escalation, and patient education skills.
  • Ability to maintain confidentiality and demonstrate professionalism, compassion, cultural sensitivity, and sound judgment.
  • Minimum personal auto liability limits of $250,000/$500,000 bodily injury split limits or a $300,000 combined single limit.
     
Preferred Qualifications
  • Experience in community paramedicine, mobile integrated health, home health, primary care, public health, chronic disease management, or care coordination.
  • Community Paramedic certification or completion of a recognized community paramedicine/mobile integrated health training program.
  • Experience working with high-risk, medically complex, geriatric, rural, underserved, or recently discharged patient populations.
  • Knowledge of motivational interviewing, health literacy principles, social determinants of health, and community resource navigation.
     
Working Conditions and Physical Requirements
  • Regular travel to patient homes and community locations may be required.
  • Work may occur in varied home environments and weather conditions.
  • Must be able to lift, carry, push, pull, and move equipment and supplies as required by the role.
  • May require standing, walking, bending, kneeling, climbing stairs, and assisting with patient movement within approved safety procedures.
  • May involve exposure to infectious diseases, bloodborne pathogens, pets, smoke, allergens, or other environmental conditions commonly encountered in home-based care.
  • Schedule may include days, evenings, weekends, holidays, on-call coverage, or flexible hours based on program needs.
