What are the responsibilities and job description for the Chronic Care Manager position at My Mobile MD?
We have an opening for Chronic Care Manager, LPN, in our exciting geriatric mobile practice focusing on wellness and disease management in the geriatric community. We do not work weekends or holidays. Our team is supportive, caring, and we have tried to build a work environment that exemplifies teamwork. You must be an LPN with experience in LTC, specifically assisted living, to qualify for this position.
Our focus is in assisted living communities. We are offering a full time position to Chronic Care Managers, LPNs, to better serve our clients medical needs. Starting pay is $55k, but with our incentive pay structure (based on your charting and documentation), our CCMs average around $78k/year.
A chronic care manager does a combination of a liaison, administrative work as well as case management. Job duties include but are not limited to; order processing, phone call communications with family and other providers, insurance processing, med reconciliation, case management, and maintaining positive relationships with building staff. You are the direct liaison between our company and the facilities we service. You will be working directly with one of our NPs who provides primary care services to our patients.
Being a mobile practice we go out to Assisted Living communities to provide services. We go to 1 community each day. Our current need is in Kokomo.
Due to the unique nature of this job, we do require our potential hires to shadow one of our current providers for 2 hours in order to make sure it is a good fit for both parties. This would require travel to the north side of Indianapolis. Please ensure you are willing to participate in the shadow prior to applying. We thank you in advance for being willing to participate.
