Clinical Rehabilitative Services Credential-Speech, Language, Hearing or Speech Pathologist Services Credential OR Masters Degree-Communicative Disorders AND Valid License issued by the Speech-Lang. Pathology/Audiology Board,
Letters of recommendation and references must be provided by current evaluating administrators. Kindly ensure all required documents are included in your application submission.
