Position Summary
The MDS Coordinator is responsible for coordinating and overseeing the completion of the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations. This role ensures accurate and timely completion of Minimum Data Set (MDS) assessments, care plans, and clinical reimbursement documentation while collaborating with the interdisciplinary team to provide quality resident care.
Essential Duties And Responsibilities
- Coordinate and complete MDS assessments according to federal and state guidelines
- Ensure accuracy and timely submission of MDS documentation
- Develop, review, and update resident care plans in collaboration with the interdisciplinary team
- Monitor resident assessments to ensure appropriate reimbursement and regulatory compliance
- Conduct resident interviews and assessments as required
- Review medical records for supporting documentation and coding accuracy
- Collaborate with nursing staff, therapy, dietary, social services, and physicians regarding resident care needs
- Participate in Medicare and Medicaid case management processes
- Monitor Quality Measures (QMs) and assist with quality improvement initiatives
- Prepare for and assist during state surveys and audits
- Educate nursing staff on documentation requirements and regulatory updates
- Maintain confidentiality and HIPAA compliance at all times
- Attend care conferences and interdisciplinary meetings as needed
- Current RN license in good standing
- Previous MDS experience preferred
- Knowledge of RAI process, Medicare, Medicaid, and PPS systems
- Strong understanding of long-term care regulations and reimbursement practices
- Excellent organizational, communication, and analytical skills
- Ability to work independently and manage multiple deadlines
- Experience with electronic medical records (EMR) systems preferred
- Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally
- Ability to use standard office and clinical equipment
- Ability to communicate effectively verbally and in writing
- Full-time
- Occasional weekend or on-call responsibilities may be required
