About the Role
We are seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to deliver home-based primary care services in Chicago region, IL. This role is ideal for a provider who is passionate about improving health outcomes through personalized care in the home or assisted living setting. You will manage a panel of patients and address both acute and chronic conditions in collaboration with a supervising physician.
Key Responsibilities
- Perform in-home comprehensive assessments and primary care visits
- Diagnose and manage chronic and acute medical conditions
- Conduct physical exams, medication reviews, wound care, lab draws, and screenings
- Identify care gaps and ensure risk adjustment and quality metric accuracy
- Collaborate with interdisciplinary care teams to ensure coordinated, patient-centered care
- Educate patients and caregivers on disease process, treatment plans, and prevention
- Maintain HIPAA compliance and accurate electronic documentation
- Travel locally across Chicago, IL s as required
Required Qualifications
- Family Nurse Practitioner (FNP), APRN, or Physician Assistant (PA)
- Active and unrestricted Indiana NP or PA license
- Board Certification (ANCC or AANP for NPs)
- Current or eligible DEA license (can be obtained post-hire)
- BLS or ACLS certification
- 2 years of NP/PA experience in a clinical or home care setting
- Knowledge of chronic disease management and geriatric/palliative care preferred
- Strong critical thinking, time management, and communication skills
- Reliable transportation and ability to travel 5 days/week within assigned areas
Work Environment
- Patient homes, apartments, and Assisted Living communities
- Frequent local travel in Chicago, IL.
- Occasional lifting (up to 10 lbs.); extended periods of sitting, documentation, and travel
- Regular interaction with patients, families, and interdisciplinary care teams
