- DESCRIPTION OF POSITION
- Responsible and accountable for managing and directing of day to day operations of Case Management. Works cooperatively with leadership of other departments to integrate clinical services, administrative and educational aspects throughout the continuum. Has 24 hour accountability. Responsible for planning, development, implementation and evaluation of the Case Management department for Morris Hospital & Healthcare Centers; provides leadership and guidance for functions related to Case Management, Utilization Review, and Social Services. Develops strategies to manage high risk patients in our community and reduce re-admissions and progression of care delays.
- QUALIFICATIONS
- Professional/Educational
- Current IL RN Licensure.
- Bachelor's Degree in Nursing required, Master's degree preferred with current Illinois RN license.
- Five years of healthcare or business experience, at least three of which have been in a supervisor or manager.
- Two years previous experience in case management/utilization review.
- Certification in Case Management preferred.
- Experience in RAC auditing / review.
- DCFS Mandated Reporter Certificate of Completion required.
