Who We Are Looking For
The Services to End and Prevent Homelessness (SEPH) has the mission to make homelessness a rare, brief, and non-recurring event. SEPH provides a full continuum of services with public and private partners, including housing stabilization, diversion from the experience of homelessness, outreach, shelter services, Healthcare for the Homeless, and permanent housing. SEPH is seeking a skilled and compassionate Licensed Certified Social Worker - Clinical (LCSW-C) to provide clinical social work services to families with minor children experiencing homelessness. This social work case manager will assess the needs of families placed in overflow shelter and support them in accessing housing focused resources needed to quickly resolve their experience of homelessness.
What You'll Be Doing
Homeless Services for Families (HSF) supports families experiencing recent housing instability due to a range of individual and systemic challenges, including limited access to affordable housing, financial hardship, unmet physical or behavioral health needs, social isolation, and barriers to childcare access.
Case Management Responsibilities Include
Education: Possession of a Master’s Degree in Social Work (MSW) from an accredited college or university that meets the criteria of graduate social work program accredited by the Council on Social Work Education
Experience: Two (2) years, to include a minimum 3,000 hours, of professional experience rendering social work services under the supervision of a LCSW-C in accordance with Maryland Board requirements.

Case Management Responsibilities Include
- Conducting intake interviews and assessing the needs of families experiencing homelessness and sheltered in county contracted overflow motels.
- Providing housing focused crisis intervention and ongoing case management services until families transition to family shelter or permanent housing.
- Developing comprehensive housing focused case plans to address barriers and support families’ rapid return to permanent housing.
- Conducting regular face to face visits with the family in overflow motel and community.
- Entering client information and service notes into County Homeless Information Management System (HMIS) database.
- Collaborating with community partners and behavioral health professionals, both within and outside the department, to identify additional resources that address the immediate crisis and support the client’s return to permanent housing.
- Processing financial assistance grants in accordance with the policies, procedures, and guidelines of the relevant funding sources, using County database system.
- Providing information, guidance, and referrals to connect clients with additional programs, services, and benefits that may offer further support.
- Participating in case reviews, unit meetings, and interdisciplinary team collaborations to plan and coordinate service delivery.
- Participating in Family Coordinated Entry System meetings to facilitate matching clients to housing opportunities.
- The completion of a Physical Exam, Drug Screen are required for this role
- A Maryland State Driver's License is required for this role.
