The Social Worker III- HIV Mental Health role fosters positive and professional interpersonal relationships with patients/families, physicians, hospital staff, community agency representatives and visitors. Provides comprehensive psychosocial assessment for identified patients and based on findings, facilitates interventions. Effectively uses interpersonal communication skills to resolve conflicts and promote healing environment. The Social Worker III must be credentialed through the CN Medical Staff Office and eligible to bill for services provided in an outpatient setting. They are responsible for independent clinical practice, providing individual, family or group psychotherapy with patients and families. The Social Worker III can provide evaluation and assessment of mental health needs including psychiatric diagnosis and treatment recommendations.

More specifically the successful candidate will provide direct and evidence-based psychological and support services to children, adolescents, and young adults living with HIV, as well as patients receiving HIV prevention care (PrEP). In this role, the mental health specialist will work with a multidisciplinary team in primarily an outpatient medical setting. The position involves providing consultative services in an outpatient clinic, psychotherapy, supportive counseling and peer support group. Additionally, the mental health specialist will be expected to engage in collaborative work with a multidisciplinary team. Other activities may include educational and advocacy initiatives as determined by the Division and program leadership.

Minimum Education

Master's Degree in Social Work (Required)

Minimum Work Experience

5 years post-MSW experience as Social Worker with treatment of patients and families managing chronic illness or traumatic experiences. (Required) And

Knowledge and experience with behavioral health documentation and billing (Required) And Experience as a Social Worker in a complex medical setting ( preferred)

Required Skills/Knowledge

Sensitivity to individuals from varied backgrounds and cultures.

Advanced clinical practice in the ability to diagnose and provide evidence based interventions with patients and families.

Knowledge of documentation and billing practices for behavioral health services.

Required Licenses And Certifications

Ind. Clinical Social Worker LICSW in the District of Columbia

LCSW-C if providing services in the State of Maryland

LCSW if providing services in the State of Virginia Upon Hire (Required)

Must be credentialed by the CNH Medical Staff Office. .

Functional Accountabilities

Ensures Effective Patient/Family Plan is Developed, Implemented and Documented

  • Facilitate interventions, based on assessment, family and staff requests, and timeframe.
  • Facilitate the child protection process of assessment, referral, plan, and implementation, if appropriate.
  • Provide follow-up services as appropriate and assesses outcome of interventions.
  • Document assessment and interventions in the patient’s medical record, in accordance with departmental standards.
  • Provide comprehensive psychosocial assessment for identified patients and families
  • Develop plan for intervention based on assessment.
  • Use range of communication skills to assist patients, families and staff to address care needs and complex issues.

Ensures Effective Multidisciplinary Communication Established Surrounding Patient/Family

  • Educate staff about psychosocial issues impacting treatment plans.
  • Organize patient care conferences when complex issues arise.
  • Participate in team meetings to address general and specific patient care issues.
  • Facilitate the discharge planning process by working with Case Management staff, nursing staff, and physicians.
  • Communicate with relevant hospital staff about patient care issues to insure continuity of care.
  • Advocate for patient and family needs within the hospital system.

Identifies, Utilizes, and Recommends Appropriate External Resources to Meet Patient/Family needs

  • Research appropriate resources to assist families to care for their children at home.
  • Share information with relevant agencies, respecting patient/family confidentiality.
  • Maintain positive and effective referral relationships with community agencies and professional organizations.
  • Advocate for patient/family needs within the community.

Removes Complex Barriers to Achieving Optimal Patient/Family Outcome

  • Intervene in complex situations involving patient care needs, communication problems, or systems issues.
  • Research appropriate solutions within the hospital or in the community.
  • Mediate conflictual situations, collaborating with other staff as appropriate.
  • Provide assistance to patients, families, and staff, while maintaining respect for boundaries and set appropriate limits

Behavioral Health Interventions

  • Formulates diagnosis for patients with acute or chronic mental health disorders.
  • Interprets data obtained from test results, social histories and interviews to determine therapeutic interventions and treatment plans.
  • Ability to form a therapeutic alliance with patients from diverse backgrounds.
  • Provides behavioral health treatment as part of the overall healthcare plan for the patient.
  • Demonstrates knowledge and skill in a variety of therapeutic tools and evidence based treatment modalities.
  • Utilizes crisis intervention skills with distressed patients and families.
  • Knowledgeable about behavioral health documentation in the EHR and for billable services.

  • Complete required continuing education courses to maintain license status.
