Role Responsibilities

Education Requirements

• MD or DO degree from an accredited institution.
• Completion of a Family Medicine residency accredited program.

Certifications

• Board Certification in Family Medicine is required.
• Must be eligible for or possess an active Arizona medical license.
• Certifications in BLS and ACLS are preferred.
