Role Responsibilities

Education Requirements
  • MD or DO degree from an accredited institution.
  • Completion of a recognized Family Medicine residency program.

Certifications
  • Board Certification in Family Medicine.
  • Active Arizona medical license eligibility (or current license).
  • BLS and ACLS certifications are preferred.
