Maternal Consortium Community Coordinator
Job Summary:
This position offers an opportunity to make a meaningful impact on the health and well-being of mothers, infants, and families in rural communities. The Maternal Consortium Community Coordinator will help strengthen local systems of care, build trusted community partnerships, and support healthier outcomes.
Serves as a liaison between healthcare providers, community organizations, local agencies, and families to improve access to care, reduce barriers, and promote healthy maternal and infant outcomes. Coordinate maternal and child health meetings and collaborative activities across the service region. Identify gaps in local services and work with partners to address barriers affecting maternal and infant health outcomes. Receive certification training as a Family Transition Specialist. No Remote Work
Salary: High $30,000s to low-to mid $40,000s annually, Negotiable with consideration for experience and qualifications.
Schedule: Full-time (40 Hours per week), Monday-Friday, with some flexibility in work hours.
Service Region: Butler, Dunklin, Mississippi, New Madrid, Pemiscot, Scott.
Job Duties:
· Establish referral pathways for Family Transition Services.
· Organize training and recruit community members for Consortium meetings.
· Represent the Maternal Consortium at Community meetings, health fairs, coalition events, and partner events.
· Facilitate meetings and communication among consortium members and community partners to ensure alignment of services and resources.
· Support the development of community partnerships that strengthen and improve the care coordination for pregnant and postpartum women and families.
· Assist in planning, implementing, and evaluating consortium goals, objectives, and community-based strategies.
Qualifications:
Education:
Bachelor’s degree in public health, social work, community health worker certification, or a related field preferred. Certified Doula preferred but not required. Relevant experience may be considered in place of formal education.
Experience:
Working in maternal and child health, community outreach, case management, public health, or rural programs preferred, 1-2 years preferred.
· Coordination of Community Partnerships and multi-agency collaboration.
· Facilitation of meetings and community outreach activities.
· Working with grant-funded programs and coalition development.
· Familiar with the service region and barriers common in rural communities.
Knowledge, Skills, and Abilities:
· Proficiency in Microsoft Office – Word, Excel, PowerPoint, Outlook
· Strong interpersonal, communication, public speaking, and relationship-building skills.
· Ability to work independently and travel in the service region (company vehicles accessible)
· Ability to work effectively with community leaders and health care providers.
· Ability to maintain documentation and meet deadlines.
· Confidentiality and HIPAA compliance standards and participant data.
