Summary:
The Community Nurse is responsible for providing oversight of the resident health care service needs. Duties and responsibilities are as followed:
Duties and Responsibilities:
- Adheres to and conveys to the Mission, Values and Principles of Mission Senior Living.
2. Is knowledgeable of the state rules and regulations regarding Assisted Living, as well as the regulations of the State Board of Nursing.
3. Maintains adequate attendance, notifies supervisor if late or unable to work in accordance with personnel policies, follows work schedule.
5. Keeps resident information confidential, respects resident’s rights, respects privacy and the right to self-determination by the residents.
6. Abides by established policies and procedures of the Community. Looks for ways to improve Community function.
- Facilitates the overall coordination of care for residents and provides oversight for the Wellness Department Team Members in accordance with the Health Service Manual and through participation in the following:
- Move In Process
- Participates in the Predetermination of Admission assessment (must be signed by RN), review of move in paperwork and initial service plan development in accordance with current policy and procedure.
- Assessment and Service Plan
- Participates in routine review of resident service plans to identify needs for additional assessments, health monitoring, or other coordination of care needs. Signs off on service plans when they are completed.
- Completes specific assessments for residents such as supportive device assessments, fall assessments and changes of condition.
- Assess, provide, and track any needed wound care as allowed by regulation to residents.
- Assess, provide and track any needed vitals and/or insulin monitoring.
- Coordinates resident’s health care in collaboration with outside health care agencies i.e. pharmacy, physician, home health, hospice etc.
- Delegation
- Provides delegation in accordance with state regulations and Community policy.
- Coordinates delegation needs with Administrator for new residents
- Maintain proper documentation related to Delegation tasks.
- Completes proper education with delegated staff to ensure they have thorough understanding of protocol.
- Infection Control
- Oversees compliance with Community policies on Immunizations, TBs, Infection Control and Infectious Disease Management.
- Team members Training and Education
- Coordinates continuing training and education as needed in accordance with state and Community policy and as identified through routine auditing of health services systems.
- Quality Assurance
- Ensures completion of and participates in routine auditing of all health services systems in accordance with Health Services Review Schedule and all other associated QA tools within.
- Move In Process
Job Qualifications:
- RN/LPN License
- CPR certification
- TB clearance
- Demonstrates knowledge and utilization of basic management skills.
- Requires minimal supervision, is a self-starter and is flexible.
- Organizes and utilizes time appropriately, sets priorities and accomplishes assigned tasks.
- Displays mature behavior in attitude and speech and action and demonstrates
consistently pleasant demeanor, tone of voice, supports the philosophy and approach used approach used by the Community.

- Is accurate, dependable, neat and thorough in assigned tasks.

Universal Precautions:
Exposure to blood/body fluids is likely.
Physical Requirements:
Has ability to do occasional heavy lifting (up to 40 pounds). Has the ability to stand for long periods of time on feet and ambulate quickly.
