- Develop and maintain relationships with residents, understanding their interests and abilities to create personalized activity plans
- Collaborate with other members of the care team to ensure seamless coordination of care and services
- Document resident participation and progress in activity programs
- Education: High School or GED
- Experience: 1 year of experience required, but willingness to learn and grow with our team
- Certifications: CNA Certificate required
