Healthcare facility part of a system looking to bring on Director of Case Management! Full Relocation!
Provides strategic and operational leadership for all care-management functions across the acute-care facility, including case management, utilization management, discharge planning, social work services, and psychosocial interventions that support safe, effective, and sustainable transitions of care. Accountable for patient outcomes, satisfaction, throughput, and care continuum utilization while driving performance that meets or exceeds regional and national benchmarks. Develops and executes specific strategic and operational plans, manages key clinical, operational, and financial metrics, and ensures services remain patient-centered and aligned.
Qualifications:
- Bachelor's degree in nursing AND a Master's degree in a health-related/science field.
- 2 years of care management experience in an acute-care setting.
- 2 years of healthcare leadership or management experience related to acute-care case management.
- Required Upon Hire: California Registered Nurse (RN) License (if applicable to role requirements).
- Required Within 30 Days of Hire: California Fire and Life Safety Card.
- Strong knowledge of acute-care case management principles and practices.
- Understanding of evidence-based practice and care-transition models.
- Knowledge of the care continuum and effective transition-of-care strategies.
- Strong problem-solving, communication, writing, and organizational skills.
- Ability to manage multiple priorities in a fast-paced healthcare environment.
- Demonstrated ability to build and maintain collaborative relationships across disciplines.
- Proficiency in Microsoft Office applications, including Word, Excel, PowerPoint, and Outlook.
