HCC Case Manager
MHMR is looking for a detailed oriented HCC Case Manager to join our team.
In this position of QMHP-CS , you will be in a direct care position with the expectation that at least 50% of his/her work time is spent face to face with the individuals that MHMR of Tarrant County serves. This position has specializations that include functioning as a Rehabilitative Case Manager (RCM, RCM-I) or a Clinical Coordinator (CC).A QMHP-CS functions as a part of the treatment team in the clinic to which he/she is assigned, and provides assessment, recovery planning, and ongoing treatment in order to assist individuals with severe and persistent mental illness to gain the skills, resources, and supports necessary to attain their individualized recovery goals.Services may be provided in the clinic or in the community.The specific job function designation (RCM, RCM-I, CC) determines what percentage of time the QMHP spends in the clinic or the community.Providing services in the community entails travel (often in the QMHP’s own vehicle) and work throughout Tarrant County, in a variety of neighborhood and home conditions.
Qualifications
Minimum Education Bachelor's Degree
Defined Education: In Health or Human Services
Acceptable Degrees: Applied Behavior Analysis, Child Development, Counseling, Early Childhood Education, Early Childhood Intervention, Education Psychology, Gerontology, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Registered Nurse, Rehabilitation & Rehab Studies, Social Work, Sociology, and Special Education
Required Experience
None
Substitutions
OR Bachelor's degree with at least 30 hours of behavioral health coursework.
Required License(s)
Valid driver’s license with an acceptable driving record.
Eligible to obtain Qualified Mental Health Professional (QMHP-CS) certification.
Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate
