We are seeking a dedicated Benefits Coordinator to join our client in Oak Park, IL. The ideal candidate will help clients navigate Medicare, Medicaid, SNAP, housing programs, and other federal/state benefits; conduct home visits and community outreach, serve as site coordinator, provide application assistance and eligibility determination, as well as build relationships with community partners and service providers
Requirements
• Bachelor’s degree in social work or related field
• 2 years’ experience, with 1 year working with older adults
• Knowledge of benefits programs and eligibility requirements
• CRS-A/D certification preferred
• Bilingual English/Spanish a plus
