Overview:

 

This nurse will be responsible for coordinating palliative care of patients in Home Care and advising/assisting those co-workers in identifying patients that need to be transitioned to Hospice. This nurse may also case manage some patientsfrom the transition period throughout their Hospice length of stay.

 

Qualifications:

  • Education: Registered Nurse currently licensed in the state of practice.
  • Experience: Two or more years related experience.
  • Certifications: BLS certified through American Heart Association.
  • Other: Current state driver's license and auto insurance.
