Job Summary
We are seeking a compassionate and highly skilled Hospice Registered Nurse (RN) Field Case Manager to join our dedicated team. This full-time or part-time role offers an exciting opportunity to provide expert nursing care and case management services in a hospice setting. As a vital member of our interdisciplinary team, you will coordinate patient care, manage complex cases, and ensure seamless communication between patients, families, and healthcare providers. Your expertise in acute and post-acute care environments will drive positive patient outcomes and enhance quality of life during their end-of-life journey.
Duties
- Conduct comprehensive patient assessments, including vital signs, physiological evaluations, and medical history reviews to develop personalized care plans.
- Manage outpatient cases across diverse settings such as nursing homes, assisted living/memory care facilities, and home environments, ensuring continuity of care.
- Collaborate with physicians, social workers, and other healthcare professionals to coordinate discharge planning and post-acute care services.
- Perform intake assessments for new patients, ensuring compliance with HIPAA regulations and documentation standards.
- Monitor patient condition through regular visits, working with patients and families to cope with changes, and adjust care plans as needed.
- Engage in utilization management by evaluating the appropriateness of services and supporting medical necessity determinations for hospice eligibility.
- Provide education to patients and families about disease processes, symptom management, medication administration, and advance directives.
- Maintain detailed documentation of patient interactions, assessments, interventions, and outcomes in compliance with regulatory requirements.
Qualifications
- Valid RN license with current state registration; additional certifications in hospice or palliative care preferred but not required
- Familiarity with HIPAA regulations and medical terminology to ensure confidentiality and accurate communication.
- Excellent patient assessment skills combined with the ability to handle complex cases involving multiple comorbidities or acute conditions.
- Strong clinical skills in areas such as education, decision making, pain and symptom management, cultural sensitivity and medication management
- Excellent communication and interpersonal skills
- Ability to provide emotional support and compassion to patients and families during difficult times
- Proficient in using electronic medical records systems
