Overview:
This nurse will be responsible for coordinating palliative care of patients in Home Care and advising/assisting those co-workers in identifying patients that need to be transitioned to Hospice. This nurse may also case manage some patients from the transition period throughout their Hospice length of stay.
Qualifications:
- Education: Registered Nurse currently licensed in the state of practice.
- Experience: Two or more years related experience.
- Certifications: BLS certified through American Heart Association.
- Other: Current state driver's license and auto insurance.
