Provide patient and family education, discharge planning, and referrals to community resources. Collaborate with physicians to establish and evaluate treatment plans, goals, and discharge criteria. Document evaluations, treatment sessions, and education per departmental standards. Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment. Ensure compliance with regulatory standards and promote high-quality, customer-focused care.
Qualifications & Skills: Masters or Bachelors in Occupational Therapy, valid OT license, BLS certification. Must possess effective communication, customer service, team collaboration, and basic computer skills. Adhere to ethical practices, maintain confidentiality, and demonstrate professionalism. Participate in ongoing professional development, mandatory education, and training activities.
Additional duties include: Attending staff meetings, ensuring safe and cost-effective use of materials, fostering teamwork, supporting infection prevention, and contributing to a positive environment.
