- Serve as the primary contact for patients and families
- Coordinate appointments, testing, treatments, and referrals
- Educate patients on diagnoses, treatment options, and resources
- Assess and address barriers to care, connecting patients with community services
- Advocate for timely and appropriate care across the continuum
- Provide hands-on patient care, including vitals, intake, and clinic procedures
- Cover other clinics as needed and perform additional duties
- Strong clinical judgment and critical thinking
- Ability to work independently in a fast-paced environment
- Calm, confident decision-making under pressure
- Clear communication with patients and care teams
- Preferred ADN or BSN; outpatient clinic experience advantageous
- Required ACLS and BCLS certifications
