- Manage a caseload of hospice patients, conducting comprehensive assessments.
- Develop, update, and implement individualized care plans focused on comfort and symptom management.
- Administer medications and treatments in accordance with care plans.
- Coordinate closely with physicians, social workers, chaplains, and aides to ensure seamless, patient-centered care.
- Educate and support families throughout the end-of-life process.
- Accurately document all care activities and interventions.
- Participate in on-call rotations to provide responsive hospice services.
- Current RN licensure with relevant hospice or palliative care experience preferred.
- Strong assessment, communication, and collaboration skills.
- Ability to provide compassionate, patient-focused care in a sensitive environment.
- Knowledge of medication administration and symptom management.
