Join us as an MDS Nurse focused on accurate and timely completion of Resident Assessment Instrument (RAI), including the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care planning in compliance with federal and state regulations.

- Assessing residents' clinical status to support individualized care planning
- Ensuring assessments reflect residents' needs and regulatory standards
- Supporting documentation and compliance with RAI guidelines, PDPM, ICD-10 coding, and Quality Measures
- Participating in the Prospective Payment System (PPS) and Case Mix systems
- Prior MDS/RAI experience strongly preferred
- Knowledge of regulatory standards, compliance, and care assessment processes
- Current LPN license with at least one year of experience and excellent assessment skills (preferred)
- CPR certification or certification within 90 days of hiring
- Ability to work independently with demonstrated leadership skills
