Physician Assistant (PA) – Nursing Home Primary Care

Location: Cortland, NY

Schedule: Monday–Friday | Flexible Schedule | Option for 4-Day Workweek

Compensation: $130,000–$150,000 Base Salary Quarterly RVU Bonuses

MedStaff Group is recruiting on behalf of a growing medical group seeking a Physician Assistant to provide primary care services within skilled nursing facilities throughout the Cortland, NY area.

This position offers autonomy, schedule flexibility, and the opportunity to build long-term relationships with geriatric patients while working alongside experienced facility and clinical teams.

Responsibilities

• Provide primary care services to nursing home residents

• Conduct routine visits, follow-ups, and chronic disease management

• Collaborate with nursing staff and facility leadership

• Participate in care management programs including CCM and RPM

• Maintain accurate documentation in SigmaCare, PCC, or similar EMR systems

Requirements

• Active New York Physician Assistant License

• Active Medicare and Medicaid Provider ID

• Experience in primary care, geriatrics, or long-term care preferred

• Skilled Nursing Facility experience is a plus

• Strong clinical judgment and communication skills
