Location: Cortland, NY
Schedule: Monday–Friday | Flexible Schedule | Option for 4-Day Workweek
Compensation: $130,000–$150,000 Base Salary Quarterly RVU Bonuses
Genesis Medical Health is seeking a dedicated Nurse Practitioner to join our growing team and provide primary care services within skilled nursing facilities throughout the Cortland, NY area.
This role offers autonomy, flexible scheduling, and the opportunity to build meaningful, long-term relationships with geriatric patients while working alongside experienced clinical and facility teams committed to delivering high-quality care.

Responsibilities
- Provide primary care services to nursing home residents
- Conduct routine visits, follow-ups, and chronic disease management
- Collaborate with nursing staff and facility leadership
- Participate in care management programs including CCM and RPM
- Maintain accurate documentation in SigmaCare, PCC, or similar EMR systems
Requirements
- Active New York Nurse Practitioner License
- Board Certification as a Nurse Practitioner (FNP, AGNP, or related specialty)
- Active Medicare and Medicaid Provider ID
- Experience in primary care, geriatrics, or long-term care preferred
- Skilled Nursing Facility experience is a plus
- Strong clinical judgment and communication skills
