Nurse Practitioner (NP) / Physician Assistant (PA) – Nursing Home Primary Care (3 Days In Facility 1 Remote Day)
Location: Stamford, NY (Catskills Region)
Schedule: 3 Days On-Site in Facility 1 Remote Day Per Week
Compensation: $130,000 – $150,000 RVU Bonus Opportunities
Genesis Medical Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team providing primary care services to geriatric patients within a skilled nursing facility located in the Stamford area of New York’s Catskills region.
This position offers a unique hybrid structure combining hands-on patient care within the facility along with one remote administrative / care coordination day per week. The role is ideal for a provider who enjoys working independently while still being part of a collaborative medical team.
Schedule
3 days per week working directly inside the nursing facility
1 remote workday per week
Flexible scheduling available
Providers must remain accessible by phone during working hours for patient care needs and coordination with facility staff
Compensation Structure
Base salary between $130,000 – $150,000
Additional RVU-based productivity bonuses
Flexible compensation models available depending on experience and schedule
Responsibilities
Provide primary care services to geriatric patients within the skilled nursing facility
Perform patient evaluations, follow-ups, admissions, and routine medical management
Coordinate care with nursing staff, specialists, families, and facility administration
Review labs, imaging, and clinical reports
Complete documentation and EMR charting in a timely manner
Utilize remote day for chart review, care coordination, telehealth follow-ups, and administrative responsibilities
Requirements
Active NP or PA license in New York
Prior nursing home / SNF or geriatric experience preferred
Strong clinical judgment and ability to work independently
Comfortable managing medically complex patients
EMR experience preferred
Reliable transportation required
