Location: Atlantic County, NJ
Salary: $120,000 – $130,000 per year
Schedule: Full-Time | Flexible Schedule
Position Overview
A growing home health care provider in Atlantic County is seeking a Nurse Practitioner (NP) to join their team and provide high-quality care to home-bound patients throughout the region. This role is ideal for a provider who enjoys working independently, building meaningful patient relationships, and delivering personalized care directly in patients' homes.
The Nurse Practitioner will manage a daily patient panel while collaborating with the broader care team to improve patient outcomes and quality of life.
Key Responsibilities
- See approximately 5–6 patients per day in a home setting
- Perform comprehensive patient assessments, including physical exams, diagnosis, and treatment planning
- Prescribe and manage medications in accordance with clinical guidelines
- Maintain accurate and timely documentation and scheduling
- Educate patients and families on disease management, preventative care, and healthy lifestyle practices
- Coordinate care with physicians, specialists, home health staff, and family members
- Monitor patient progress and adjust treatment plans as needed
- Ensure compliance with all applicable clinical and regulatory standards
Qualifications
- Active New Jersey Nurse Practitioner license
- DEA certification
- Malpractice coverage
- Prior Nurse Practitioner experience in home care, primary care, nursing homes, urgent care, clinic, or emergency room settings preferred
- Strong clinical judgment and ability to work independently
- Excellent communication and interpersonal skills
- Reliable transportation and willingness to travel throughout Atlantic County
